中文摘要:目的幽门螺杆菌(Hp)可能通过细胞毒素相关蛋白A(cagA)参与特发性血小板减少性紫癜(ITP)发病。比较ITP患者不同血小板分组血小板抗体IgG(PA-IgG)、CagA定量,Hp、CagA阳性率,探索CagA与ITP相关性。方法 ITP组(93例)总体Hp、cagA阳性率与健康组(49例)、非血液系统疾病血小板减少组(55例)比较。ITP组按血小板数值分为PLT≤20×109/L,20×109/L0.05。ITP三组间PA-IgG、CagA定量,CagA阳性率比较,血小板低组,PA-IgG、CagA定量,cagA阳性率高,差异有显著性,P<0.05;50×109/L≤PLT组Hp阳性率明显低于PLT≤20×109/L组,P<0.05;PLT≤20×109/L组与20×109/L0.05。结论 Hp参与ITP发病,ITP血小板计数与Hp感染率不完全一致,与PA-IgG、cagA存在相关性。提示,HP可能因CagA致PA-IgG升高,导致血小板降低。

外文摘要:【Objective】 Helicobacter Pylori(Hp) involved in pathogenesis of Idiopathic Thrombocytopenic Purpur(ITP) by Cytotoxin associated protein A(cagA).According to the platelet values,ITP patients were divided into three groups.Through the comparison between the groups PA-IgG,CagA quantitative,Hp,CagA-positive rate,to explore the relation between Cytotoxin associated protein A and ITP.【Methods】 contrast Hp and cagA-positive rate of 93 patients with ITP and health group(49 cases),non-hematological diseases platelets less Group(55 cases).To give 93 patients with ITP,divide them into three groups: PLT≤20×109/L,20×109/L〈PLT〈50×109/L,50×109/L≤PLT.Rapid urease test(RUT) and 14C urea breath test(UBT) is detected by Hp.Enzyme linked immunosorbent assay method survey the PA-IgG and the cagA levels of ITP patients.It is quantitative comparisons between groups of patients with ITP PA-IgG,cagA and Hp,cagA-positive rate.【Results】 Hp,cagA-positive rate of the ITP group are significantly higher than the health group and the non-blood diseases platelets less group(P 〈0.05);Hp,cagA-positive rate of the health group are not significantly different with the non-blood diseases platelets less group(P 〉0.05).For patients with ITP,Lower platelet group,PA-IgG,CagA quantitative and cagA-positive rate are higher,Comparative differences between the groups significantly(P 〈0.05);The Hp positive rate of the 50×109/L≤PLT group is significantly blower than the PLT≤20×109/L group(P 〈0.05);The Hp positive rate is not significant difference between PLT≤20×109/L group and 20×109/L〈PLT〈50×109/L group,between 20×109/L〈PLT〈50×109/L group and 50×109/L≤PLT group(P 〉0.05).【Conclusion】 Hp involved in pathogenesis of ITP.ITP patient platelet count and Hp infection rate is not entirely consistent.The correlation exists in ITP patient platelet count and PA-IgG,cagA.HP may induce the rise of PA-IgG by CagA that Leads to ITP patient’s platelet reduce.
