中文摘要:目的探讨甲状腺功能正常人群游离甲状腺素(FT_(4))和促甲状腺激素(TSH)水平与贫血的相关性。方法回顾性收集2023年1至6月绍兴文理学院附属医院健康体检的1037名受检者资料,按FT_(4)水平均分为3组,按TSH水平均分为3组。分别比较不同FT_(4)、TSH水平3组间性别、年龄、体重指数、收缩压、舒张压、游离三碘甲腺原氨酸(FT_(3))、FT_(4)、TSH、红细胞、血红蛋白、红细胞平均体积(MCV)和贫血患病率的差异,并分析贫血的细胞学分类和影响贫血患病的因素。结果与低FT_(4)水平组比较,中、高FT_(4)水平组FT_(3)、红细胞、血红蛋白、MCV水平均明显升高(均P<0.05),贫血患病率均明显降低(均P<0.05),高FT_(4)水平组TSH水平明显降低(P<0.05)。与中FT_(4)水平组比较,高FT_(4)水平组FT_(3)、红细胞、血红蛋白均明显升高(均P<0.05),TSH水平明显降低(P<0.05);贫血的细胞学分类以小细胞性贫血为主。与低TSH水平组比较,中、高TSH水平组FT_(3)、FT_(4)、红细胞、血红蛋白水平均明显降低(均P<0.05),与中TSH水平组比较,高TSH水平组FT_(3)、FT_(4)、红细胞、血红蛋白水平均明显降低(均P<0.05),MCV和贫血患病率比较差异无统计学意义(P>0.05)。logistic回归分析显示,校正性别、年龄后,低FT_(4)是甲状腺功能正常人群患贫血的独立危险因素(P<0.05),TSH不是贫血患病的独立影响因素(P>0.05)。结论在正常甲状腺功能人群中,低FT_(4)者的红细胞和血红蛋白降低,贫血患病率增高;低FT_(4)是甲状腺功能正常人群患贫血的独立危险因素。

外文摘要:Objective To explore the correlation between thyroid function and anemia in individuals with normal thyroid function at different levels of free thyroxine(FT_(4))and thyroid stimulating hormone(TSH).Methods Retrospective collection of data from 1037 subjects who underwent health examinations at the Affiliated Hospital of Shaoxing University from January to June 2023,divided into three groups based on FT_(4)levels and three groups based on TSH levels.Compare the differences in gender,age,thyroid function,red blood cells,hemoglobin,mean corpuscular volume,and anemia incidence among the three groups,and analyze the cellular classification of anemia and the factors affecting anemia incidence.Results Compared with the low FT_(4)level group,the levels of FT_(3),red blood cells,hemoglobin,and mean corpuscular volume(MCV)were significantly increased in the medium and high FT_(4)level groups(all P<0.05),the incidence of anemia was significantly reduced(all P<0.05),and the TSH level was significantly reduced in the high FT_(4)level group(P<0.05).Compared with the medium FT_(4)level group,the high FT_(4)level group showed significant increases in FT_(3),red blood cells,and hemoglobin(all P<0.05),while TSH levels were significantly reduced(P<0.05);the cytological classification of anemia was mainly based on microcytic hypochromic anemia.Compared with the low TSH level group,the FT_(3),FT_(4),the levels of red blood cells and hemoglobin were significantly reduced(both P<0.05).Compared with the group with medium TSH levels,the group with high TSH levels showed FT_(3),FT_(4),the levels of red blood cells and hemoglobin were significantly reduced(both P<0.05),while there was no statistically significant difference in the incidence of MCV and anemia(P>0.05).Logistic regression analysis showed that after adjusting for gender and age,low FT_(4)was an independent risk factor for anemia in individuals with euthyroidism(P<0.05),while TSH was not an independent influencing factor for anemia(P>0.05).Conclusion In the population with euthyroidism,individuals with low FT_(4)have a decrease in red blood cells and hemoglobin,and an increased incidence of anemia;low FT_(4)is an independent risk factor for anemia in euthyroid individuals.
