中文摘要:目的探讨肺泡灌洗液(BALF)中乳酸脱氢酶(LDH)、细胞角蛋白19片段(CYfra21-1)、降钙素原(PCT)水平联合检测诊断肺癌的临床价值。方法选取2017年7月~2018年7月我院门诊及住院行电子支气管镜检查的患者98例为疾病组,分为肺癌组(58例)和肺部良性病变组(40例)为研究对象,选取同期收治的体检健康者100例作为健康组,采用酶联免疫法(ELISA)检测所有入组对象术前血清LDH、CYfra21-1和PCT水平,检测疾病组肺泡灌洗液中LDH、CYfra21-1和PCT水平,分析肺泡灌洗液中的LDH、CYfra21-1和PCT单独和联合检测在诊断肺癌中的意义。结果疾病组血清LDH、CYfra21-1和PCT水平均高于健康组(P<0.05);肺癌组血清LDH、CYfra21-1和PCT水平高于肺部良性病变组(P<0.05);肺癌组BALF中LDH、CYfra21-1和PCT水平均高于肺部良性病变组。ROC曲线分析显示,BALF的LDH、CYfra21-1和PCT联合检测肺癌的曲线下面积(AUC)为0.866,高于血清中LDH、CYfra21-1和PCT三者联合检测AUC为0.753。结论相比于血清指标,BALF中的LDH、CYfra21-1和PCT联合可以为肺癌患者提供更准确的诊断。
外文摘要:Objective To investigate the clinical value of combined test of the levels of lactic dehydrogenase(LDH), cytokeratin fragment 19(CYfra21-1) and procalcitonin(PCT) in bronchoalveolar lavage fluid(BALF) in the diagnosis of lung cancer. Methods 98 patients who came to the Outpatient Department and hospitalized for electronic bronchoscopy from July 2017 to July 2018 were selected as the disease group, which were divided into the lung cancer group(58 patients) and the benign lung lesions group(40 patients) as the study objects, while 100 healthy examined subjects admitted and treated during the same period were selected as the healthy group. The preoperative serumLDH, CYfra21-1 and PCT levels of all the included subjects were detected by euzymelinked immunosorbent assay(ELISA), and the levels of LDH, CYfra21-1 and PCT in bronchoalveolar lavage fluid in the disease group were detected to analyze the significance of LDH, CYfra21-1 and PCT in bronchoalveolar lavage fluid alone and in combination in the diagnosis of lung cancer. Results The serumLDH, CYfra21-1 and PCT levels in the disease group were higher than those in the healthy group(P<0.05);the serumLDH, CYfra21-1 and PCT levels in the lung cancer group were higher than those in the benign lung lesions group(P<0.05);the levels of LDH, CYfra21-1 and PCT in BALF in the lung cancer group were higher than those in the benign lung lesions group. ROC curve analysis showed that the area under the curve(AUC) for the combined test of LDH, CYfra21-1 and PCT in BALF was 0.866, which was higher than that for the combined test of LDH,CYfra21-1 and PCT in serum(0.753). Conclusion The combination of LDH, CYfra21-1 and PCT in BALF can provide a more accurate diagnosis for patients with lung cancer than serum indexes.
