中文摘要:目的探究可溶性人髓系细胞触发受体1(sTREM-1)和可溶性血红蛋白清道夫受体(sCD163)在肺癌患者术后肺部感染中的表达及临床意义。方法选取胸外科接受手术治疗的肺癌患者122例,以术后是否发生肺部感染分为感染组47例和非感染组75例,分别检测两组患者术前1 d、术后1 d、术后3 d血清中sTREM-1、sCD163水平,采用受试者工作特征曲线(ROC)评估sTREM-1、sCD163对肺癌术后并发肺部感染的早期预测价值。结果47例感染组患者共分离出43例病原菌株,其中革兰氏阴性菌34株,以肺炎克雷伯菌为主;革兰氏阳性菌9株,以金黄色葡萄球菌为主。两组患者术前血清中sTREM-1、sCD163水平比较,差异均无统计学意义(t分别=1.15、0.20,P均>0.05),而术后1 d及3 d感染组血清中sTREM-1、sCD163水平明显高于非感染组,差异均有统计学意义(t分别=22.43、21.95、8.34、8.07,P均<0.05)。ROC曲线分析显示,sTREM-1、sCD163预测肺癌术后肺部感染的曲线下面积分别为0.82、0.79。结论肺癌患者术后并发肺部感染时血清中sTREM-1、sCD163水平明显上升,对肺癌术后肺部感染的早期诊断具有一定价值。

外文摘要:Objective To explore the expressions and clinical significance of soluble human myeloid cell trigger receptor 1(sTREM-1)and soluble hemoglobin scavenger receptor(sCD163)in postoperative lung infections in patients with lung cancer. Methods A total of 122 lung cancer patients who underwent thoracic surgery were selected and divided into the infected group with 47 cases and the non-infected group with 75 cases according to whether lung infection occurred after the operation.Serum levels of sTREM-1 and s CD163 were detected at 1 day before surgery,1 day and 3 days after surgery. Receiver operating characteristic curve(ROC) was used to evaluate the values of sTREM-1 and sCD163 in early diagnosis of postoperative lung infection in patients with lung cancer. Results A total of 43 pathogenic strains were isolated from 47 patients in the infection group,of which 34 were gram-negative bacteria,mainly Klebsiella pneumoniae,and 9 were gram-positive bacteria,mainly Staphylococcus aureus. The preoperative serum levels of sTREM-1 and sCD163 was not statistically different between the two groups(t=1.15,0.20,P>0.05).The serum levels of sTREM-1 and sCD163 in the infection group at 1 day and 3 days after surgery were significantly higher than those in the non-infected group(t=22.43,21.95,8.34,8.07,P<0.05).ROC analysis showed that the areas under the curve of sTREM-1 and sCD163 for predicting postoperative lung infection were 0.82 and 0.79,respectively. Conclusion The serum levels of s TREM-1 and s CD163 in patients with lung cancer complicated by lung infection after surgery are significantly increased,which has certain value in the early diagnosis of lung infection after lung cancer surgery.
